Znalazłem następujący kod i działa dobrze, ale chcę usunąć niepotrzebne pola. np. jak mogę usunąć pola "Nazwisko" i "E-mail" bez zerwania działającego kodu?

Z jakiegoś powodu to nie działa. Po prostu pusta strona po naciśnięciu przycisku Wyślij. I nie ma nowej skrzynki pocztowej. –
